چکیده

Background/introduction Thermal ablation techniques using heat conduction (e.g. radiofrequency ablation) are sensitive to the cooling effect of blood flow, the heat sink effect. Thermal ablation by high intensity ultrasound guided by magnetic resonance imaging (MRgHIFU), is less dependent on heat conduction, and produces sharply delineated ablated volumes. The aim of this study was to ablate adjacent to large hepatic and portal veins, to study the heat sink effect on the ablation cells, as well as vessel wall patency. A secondary aim was to study features of second sonication cycles.

MRgHIFU safety issue: Validation of targeting accuracy using an MR compatible ballistic model

Introduction Volumetric ablation has been investigated in vivo demonstrating that a large and uniform ablation zone can be obtained rapidly (1-2). However, there exists no experimental demonstration to show that such volumetric lesion is indeed centered on a specific predefined target in 3D, i.e. no “gold standard” proof of absence of thermal buildup drift during the sonication (3). The effecti...

Respiratory-Gated MRgHIFU in Upper Abdomen Using an MR-Compatible In-Bore Digital Camera

OBJECTIVE To demonstrate the technical feasibility and the potential interest of using a digital optical camera inside the MR magnet bore for monitoring the breathing cycle and subsequently gating the PRFS MR thermometry, MR-ARFI measurement, and MRgHIFU sonication in the upper abdomen. MATERIALS AND METHODS A digital camera was reengineered to remove its magnetic parts and was further equipp...

MRgHIFU safety issue: Multi-layer protection against tissue-to-air interface heating

Introduction. MRgHIFU is a hybrid technology which aims to offer efficient and safe thermal ablation of targeted tumors or other pathological tissues, while preserving healthy surrounding structures unaltered. Theoretically MRgHIFU has no limitation regarding lesion size [1]. The main challenge is to avoid near and far field heating [2]. We demonstrate here that reflection at tissue-to-air exit...

Sonalleve MR-HIFU with DISC Philips MR-guided High Intensity Focused Ultrasound with Direct Skin Cooling

High Intensity Focused Ultrasound guided by Magnetic Resonance Imaging (also known as MR-guided HIFU, MRgHIFU, or MRgFUS) is a therapy technique which uses focused ultrasound waves to heat and coagulate tissue deep inside the body without damaging intervening tissue and offers an novel, non-invasive treatment option for solid tumors. The Philips Sonalleve MR-HIFU system, initially released in 2...

Preventing far-field bone-reflection of HIFU beam by selective elements de-activation is a sub-optimal approach

Introduction. MRgHIFU is a hybrid technology which aims to offer efficient and safe thermal ablation of targeted tumors or other pathological tissues, while preserving the normal surrounding structures unaltered. Theoretically MRgHIFU has no limitation on lesion size [1]. The main challenge is to avoid near and far field heating [2]. We demonstrate here that beam reflection on bones is a major ...
